Support Coordination (SC) services help individuals with developmental disabilities and their families navigate and access the resources they need to live fulfilling lives. A Support Coordinator works closely with the individual to create, implement, and monitor a person-centered service plan, connecting them with community programs, healthcare, housing, employment, and other supports that promote independence and well-being.
The Support Coordinator is responsible for creating and updating the Individualized Service Plan (ISP) in collaboration with the individual, their family, and other team members chosen by the individual. They also oversee the ongoing monitoring of the services outlined in the ISP to ensure they are being properly delivered.
(DSP) services provide personalized assistance to individuals with developmental disabilities to help them live as independently as possible. DSPs support with daily living activities, community involvement, skill-building, and behavioral or emotional needs, while promoting dignity, inclusion, and a higher quality of life.
Direct Support Professional (DSP) helps individuals with developmental disabilities live more independently and participate fully in their communities. DSPs provide support with daily living needs, skill-building, and emotional well-being while ensuring safety and dignity. DSPs play a vital role in promoting inclusion, respect, and a higher quality of life.

Home health aide services provide in-home support with daily living activities such as bathing, dressing, grooming, light housekeeping, and meal preparation. also assist clients with mobility, medication reminders, and companionship to ensure safety, comfort, and improved quality of life at home.
Home Health Aide (HHA) provides in-home care and support to individuals who need assistance with daily living activities. Their role includes helping with personal care such as bathing, dressing, grooming, and mobility, as well as assisting with meal preparation, medication reminders, and light housekeeping. HHAs also offer companionship and emotional support, ensuring the client’s safety, comfort, and overall well-being while promoting independence at home.
Private Duty Nurse (PDN) services provide one-on-one, individualized nursing care for clients with complex medical needs in their home or community. These services may include medication administration, monitoring vital signs, managing medical equipment, providing skilled nursing interventions, and coordinating care with healthcare providers to ensure the client’s safety, health, and well-being around the clock.
The role of a Private Duty Nurse (PDN) is to provide specialized, one-on-one nursing care to clients with complex medical needs in their home or community. They monitor health conditions, administer medications, manage medical equipment, and perform skilled nursing procedures. PDNs also educate and support clients and their families, ensuring safety, promoting independence, and maintaining overall health and well-being.
Tender Hand Healthcare Services provides compassionate Behavioral Health Care and Psychiatric Mental Health Services designed to support emotional well-being and mental health. Our qualified professionals deliver personalized care, therapy, and psychiatric support to help clients manage challenges, build resilience, and improve their overall quality of life.
Psychiatric-Mental Health/Behavioral services provide professional care and support for individuals experiencing mental health, emotional, or behavioral challenges. These services include assessment, therapy, counseling, behavior management, and medication support, all designed to help clients develop coping skills, improve emotional well-being, and achieve a higher quality of life.
Psychiatric-Mental Health/Behavioral Health Nurse Practitioner (PMHNP) is to provide expert, individualized care for clients experiencing mental health or behavioral challenges. They assess, diagnose, and treat psychiatric conditions, develop personalized treatment plans, and may prescribe and manage medications. PMHNPs also provide therapy, counseling, and support to help clients build coping skills, manage symptoms, and improve overall emotional and mental well-being. Their goal is to promote recovery, resilience, and a higher quality of life for every client they serve.
