What is support coordination services?

Support Coordination (SC) services help individuals with developmental disabilities and their families navigate and access the resources they need to live fulfilling lives. A Support Coordinator works closely with the individual to create, implement, and monitor a person-centered service plan, connecting them with community programs, healthcare, housing, employment, and other supports that promote independence and well-being.

Role of a Support Coordinator:

The Support Coordinator is responsible for creating and updating the Individualized Service Plan (ISP) in collaboration with the individual, their family, and other team members chosen by the individual. They also oversee the ongoing monitoring of the services outlined in the ISP to ensure they are being properly delivered.

What is Direct Support Professional (DSP) services?

(DSP) services provide personalized assistance to individuals with developmental disabilities to help them live as independently as possible. DSPs support with daily living activities, community involvement, skill-building, and behavioral or emotional needs, while promoting dignity, inclusion, and a higher quality of life.
